Parent Portal

Note: The Parent Portal is used to submit several forms required throughout the year, as well as manage other items such as payments and registrations for a variety of student/family needs.

Among the forms that may be submitted via the portal, you will find:

  • College Visitation Forms (Juniors & Seniors)
  • Absent Note
  • Late Note
  • Senior Trip Payments

Physicals for 2024-2025

Please be attentive to the following requirements for the upcoming 2024-2025 school year.

  • All students who intend to participate in a sport or Blue and Gold must have a physical completed by a physician dated on or after June 1, 2024 and submitted before they can begin trying out for a Fall sport in August or the first day of school if not playing a Fall sport.
  • Students new to Kellenberg Memorial, as well as all students entering grades 6, 7, 9, and 11, must provide a physical by the first week of school. Ideally, the physical will be dated on or after June 1, 2024, but to be valid, the physical cannot be dated any earlier than September 4, 2023);
  • Students entering grades 8, 10, and 12 who are not new to Kellenberg Memorial do not require a new physical unless they plan to participate in sports or Blue and Gold.

New York State requires the completion of the NYS Health Interval Form for all student-athletes for each of the three athletic seasons when a physical has not been completed within 30 days of the start of that season. The KMHS Physical Form and the NYS Health Interval Form are available on the KMHS website on the Parents tab and must be submitted via the Magnus Health portal.

Immunizations

Please note that New York State requires all students to remain up to date with their immunizations. Immunization updates must be provided by the first day of school. All forms are to be submitted through the Magnus Health Portal. More information on submitting the forms will be provided at the end of this school year.
